For minors, this Digital Parental Consent Form has to be filled in and signed by the parents or legal guardian, the minor and the responsible person during the specific championship. the EBL will automatically receive a copy and the signing person will also receive a copy. Please note that this Parental Consent Form is to be filled in/signed for each Championship entered by a minor.
